Após a autenticação o JotForm não reconhece os campos requeridos para proseguir com a integração.

May we ask for the URL of the form you are referring to? I checked the forms in your account and none of them seem to be integrated with ZOHO crm.
If you are referring to this form (http://www.jotformz.com/form/53416002603643), the fields might not be recognized because the form is in a different language. Please remove the previous integration and try making your form multilingual by following this guide: How to make your forms multilingual? Then you can try reintegrating the form with zoho crm again.
