Hello!
How can I create the form with 2 steps?
Step 1 - A person fills a couple of fields and presses submit button. After that the second form loads (step2)
Step 2 - A person fill another couple of fields and presses the next submit button. The date of step1 and step2 are gathered into one date and belongs to the certain person.
Is it possible?

Yes that is possible, you can check this guide on how to pass the data from form 1 to form 2 so that the same date and same name can be filled out in form 2 without the need to type the text again: http://www.jotform.com/help/351-How-to-automatically-pass-form-data-to-another-form.
Let us know if you need further assistance on this.

Field1
Field2
Field3
Field4
Field5
Submit
into this:
Field1
Field2
Submit1 (or go to the next page)
And then on the next page:
Field3
Field4
Field5
Submit2
I think I saw this option before.

Hello askmeabout,
Well you can create one single form and then add "Page Break" field to separate the forms in two page. Please check the screenshot below:
Hope this helps.
Do get back to us if you have any questions.
Thank you!
