What exactly do you mean by "degraded"?
I took a look at your form but currently it seems okay so I'm unsure of what the problem is as of right now.

Thank you for your response.
The background of some sections had disappeared so that the white text couldn’t be seen. Also the included photo had moved to the right and was partly outside of the right hand frame edge.
In the meantime one of my colleagues has managed to fix the problem.
It seems to be ok now.
