How do I make a duplicate set of fields?

    As you can see here:

    Here was what I am trying to do:

    I want the fields above the button that says "Do You Have Another Facility or Practice?" to duplicate when the "Do You Have Another Facility or Practice?" field is checked.

    Is this possible? Or what is the best work around for what I am trying to acomplish?

    I understand that you want to duplicate set of fields so that you can collect multiple entries for the field. I believe that you are looking for one of the following widgets that can help you in getting multiple entries for a field.

    Otherwise, you'll need to duplicate the fields manually and add conditions to display them when the checkbox is checked. The following guide can help you in setting up conditions on your form.

    ok thanks! I attempted to use this one: http://widgets.jotform.com/widget/configurable_list

    However, is it not responsive? My options extend way to far right of my embedded frame on my website designed by adobe muse.

    The widget configurable list loads on an iframe and not responsive. In this case, at the moment you may need to duplicate the fields and add conditions to hide/show them. You may use the field manager wizard to duplicate the fields easily on your form.

    You can get into this wizard by right-clicking on any field on your JotForm and select Manage Multiple fields from the quick menu. You can then also duplicate the conditions by selecting respective fields.
