Wish to find out if I can get a receipt when signing up for Jotform Premium?
Also, is your sign up based on calendar month? or 30-days?

You can get a receipt when upgrading to Premium we can send it out via email. Upon sign up you can choose from monthly or yearly subscription. When you sign up on a monthly basis the billing cycle starts on the date that you upgrade however the account quota (submission limits) always resets on the first day of every month.
