Does Jotform have any recommendations about which browers work best with jotform?
Last year some people reported problems with Chrome when they were completing:
I'm advising people to make sure that they are using the most recent version of their browser. Is that enough?

Please be noted that JotForm works with almost all browser without any problem. Though we recommend chrome browser for form builder but the form itself should work in all recent browsers without any problem.
I did test your form and I was able to submit it successfully without any problem. Please check the screenshot below:
The only issue I see in your form is just above the submit button which I have highlighted in the screenshot above. You have added a "Text" field and a widget which is not being used. It is safe to delete them. Please check the screenshot below:
