We have converted a paper/fillable pdf waiver form that we use in our background screen process into a jotform. The paper version of this form asks for a social security number, and we are required to input a SSN when we complete the background screen online.
Can you please advise how we can add the SSN field to this form in a secure way that does not violate your terms of service?

Collecting Sensitive Information. You may not use the JotForm to collect certain types of sensitive information, including but not limited to credit card information and any type of login credentials. You may collect some sensitive information such as social security numbers or driver’s license numbers, but you are required to use best security practices of JotForm including SSL and Encrypted Forms features. You are solely responsible for compliance with any data protection and privacy laws and rules applicable to the sensitive information.
As a rule, you should not capture multiple sensitive information using the form. If you need to collect SSN, then make it minimal to collect only at least another data like Full Name (do not include email or other info anymore about the user)
Because if your form collects data information collectively like Name, Email, Phone, Address and SSN... the Phishing Filter will perceive it as a Phishing form and will be suspended immediately.
Phishing. You agree that Company may terminate your JotForm Service immediately if a form's purpose : sensitive credit card information, social security numbers, user login credentials, or other sensitive personal information. All these activities are considered as Phishing and any account along with the violating forms will be suspended immediately.
