The registrants using our form are getting quite alarmed because the autoresponder is sending out everything BUT the dancer name and birthdate. I can see the info in my submissions on the form so I have the info, however, the parents of the dancer's are being made to be worried unnecessarily. How do I fix? When I test it shows the fields in there, however, not working!!!

I cloned your form and inspect the email alerts. I can see that you have 2 autoresponders, one is KCHDA 2016 Competition and the other is Registration Confirmation.
Registration Confirmation has the correct field IDs for dancer full name and birth date.
However, KHCDA 2016 Competition has incorrect dancer full name field ID.
So you need to edit the autoresponder email and check the field IDs are correct. You can also just delete the autoresponder and create a new one. This will automatically capture all the field IDs in your form, though you need to setup the Sender and Recipient settings again.
