A form can't preview / is reverting back

Hi there, we have a form called 'LAS Signup Form' but for some reason we can't publish or save the form it just goes to a page not found link. 

Weirdly, it keeps reverting back to an earlier version as well, so the form title changes, there is a missing image field and some of the filed settings are reverting back too. 

Is there something we're doing wrong? 

There was a database replication problem. It should be fixed now. Please see if the changes are live now.
