Form submissions are not being automatically added to my Google Sheets.
I tweaked the sheet slightly by adding 2 new columns for editing purposes and 2 new rows with instructions on how to view the answers:
My Google Sheets remains empty.

The reason why google spreadsheets integration stopped populating is due to the fact that you modified your form so that broke the spreadsheet functionality since the changes caused it to fall out of sync with your data.
To fix it all you have to do is remove the integration and reapply it to rebuild it with the new fields that you had added in when you changed it then it should begin working again.
Additionally, I ran a test and I can confirm that our google spreadsheets integration is functioning properly today as far as I know.
