I can view my form in edit mode, but when I try to view it the program reports that the file cannot be found and that it has been disabled. Why has it been disabled and how can I get it up and running again?

First of all, we are sorry for inconvenience this may caused you. We were having problem in our system. This issue should be fixed now. I have checked your forms and I have not found Phishing, which is against our Terms of Use so I have reactivated your account.
