I created a new form and published it. (Life Insurance Application)

The URL link keeps saying it is disabled

You form and account was suspended by our automatic phishing detector.  We do not allow for the collection of sensitive data directly through forms.  Things such as drivers license and social security numbers are not allowed.

I removed those fields from your form and activated your account.  Your forms should be available once again.
