I have checked your account and it is on auto-suspension status. We have an Anti-Phishing system that marks forms detected or suspected of phishing activity. I have reactivated your account for now and whitelisted your form. I have noticed that the form is created using a different language. You may try to re-create the form in English, then make the form multilingual by following the instructions in this guide, How-to-make-your-forms-multilingual. After that, make that other language as the default language of your form (also explained in the guide).
