Want to build a form and attach to therapy sites

I need help please to know what is the correct way to set this up

Most basic fields can be added using our form builder.  If there are any special fields you require, we likely have a widget that can fill the need.

As for adding your form to a webpage, the best way to do so is adding your forms iFrame code to your page:
