How do I set up the Conditions Page? I don't understand it at all. What I want to do is have the form sent to my email address and set up a thank you reply

If I understood you correctly you want to receive email with submitted data when your users submit your form and you also want that email with submitted data is sent to your users.
To achieve this all you need to to is to set email Notification and Autoresponder email inside of your form.
Email notifications will be sent to your email and Autoresponder email will be sent to your users.

You can, but you do not have to create conditional emails. I have inspected your form http://www.jotform.us/form/60596406830155 and I saw that you have set conditional email Notification. However that condition will not be met because you have set conditions based on text fields. I would suggest you to delete or disable that condition.
