How do i create this kind of matrix?

    I am unable to add the last column (a customized column) onto the grid. This column should contain a textarea for explanations for each of the questions on the matrix (see the attachment for a sample). I would like to use the matrix because the presentation is better than breaking them down into single questions.Can anyone assist me her?

    The current "Matrix" field found under our "Survey Tools" section can only have one type of input field, not a combination of radio button and text box or text area fields.

    However, we do have a widget that allows you to have this kind of table. You can use the "Configurable List" widget. You can learn more about this on this guide: http://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget. With that, just set the specific number of rows and redesign the widget to look like a table using CSS code.
