The picture shows the edit mode of Google Form using the GRID question type. The second picture below this one shows the live form. Note how I can label each column with varying descriptions of each column choice.
Here is a link to the actual Google Form.
I would much rather use Jot Form to do this... It is far more powerful than Google Form but cannot find a way to use thew GRID question type.

In addition to the Matrix available in the form builder tools:
Depending on your needs, one of these should be able to do accomplish what you are looking for.
