How do I change the name of the link for my form

To change the link of form to custom link, open the form in Form Builder and select 'Publish', click on 'Link' then the edit icon, from  there you can change your form link to what is desired.

I tried to change the name of the link but it will not remove my name.  It only allows me to change the ending.  How can I take my name off of the link? 

You cannot change that part of the link. The custom URL feature allows you to have a link with your username and a custom text at the end. Here's a guide about it: https://www.jotform.com/help/261-How-to-create-a-Custom-URL-of-a-form
