How to add a form to a Facebook?

    i'm trying to add a 2nd form to facebook...

    having trouble.. the link included is the form i want to add to my facebook page

    SO I GOT THE iframe on my facebook page.. but I can't figure out how to put the other jotform page on there... now I have 2 of the same forms on my page...

     

    can you help me.

    You may also embed your form in Facebook using our embed wizard, please follow this guide to get further details: http://www.jotform.com/help/22-Adding-Form-to-Facebook-Pages.
