    I'm checking one of your forms and it is being displayed properly in IE11 and Microsoft Edge. I'm using Windows 10 OS.

    May we know if your form is embedded on a website? If it is, could you please share us the web page link so that we can better check it. You might need to use our iFrame embed code to avoid script conflict that may occur between your website and your form's resources.

    I was able to view your form in this website page http://centomiglia.be/deelnemer.html. However, I did see that it disappears at some page load.

    Could you please try re-embedding the form using the iFrame embed code? It seems like the script code is failing in your website at some point.

    Here's a guide that you can follow on how to get our iFrame embed code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code. Paste it on the HTML element where you have originally place your script code. Make sure to remove the previous embed code before using the iFrame.

    i think i solved the problem with source mode. not the i-frame

    I am able to see it right now in windows professional.

    i asked others to check also what they see now on their PC
