How can I create a form for write in and multiple choice payment options

I know this is something simple but for some reason I am not able to figure it out.  I am using the payment module.  I want to offer people the option to make a donation.  I want to list some options, like $200, $500 & $1,000 but also give them the option of writing in the amount they want to pay.  This is a one-time payment option.  I have created a confirmation letter and it will be pulling in the amount they they are going to pay and putting it in the letter.  Those amounts in the letter will either reflect what they chose in the multiple-choice option or in the write-in fields. 

How do I go about creating this?

If I understood you correctly, you want to allow your users to select some donation amount or write in amount they want to donate. Payment donation field only has custom amount option.

As workaround you can add radio button field to your form and set option to $200, $500 & $1,000 and also turn of Allow other option, so that your users can also write in amount they want to donate. Then you need to set calculation values for that field.

Then add form calculation field and pass value from radio button field to calculation field.

Then pass the value from calculation field to your Paypal donation field.
