I am trying to create a form that looks like the link i pasted below. It is a form on jotform but I need to incorporate this exact form and calculation into my form i have. Please look at the link and let me know how I can do this EXACT THING!

Since the original creator of the form allows for their forms to be cloned, you can simply try cloning their form into your own account:

When it comes to explaining what they did, they simply used:
1. Driving Distance widget (How to Add a Widget to your Form)
2. Form Calculation widget, which they have configured to calculate as "Driving distance widget times 0.8":
This guide has a little more information about how to perform calculations in your form.
