Hello,
I am trying to create a limousine reservation form. When the customer inputs their pick up location and drop off location, the form will automatically calculate the distance in miles.
I currently have my distance in miles formula as (Number Miles x $3.89).
Then, the form automatically will fill in the total rate (for example, 3 miles x $3.89 = $11.67)
My question is, I also need the customer to select a specific vehicle type. I have 6 different vehicles to choose from:
Sedan, SUV, Luxury SUV, Limousine, and SUV Limousine
I want the image of each vehicle to now "show up" with a rate for each vehicle.
For example,
Sedan Pic = $11.67
SUV Pic = $15.97
Limousine Pic = $89.00
etc.

You can perform calculation in your form fields using the form calculation widget. Here's a guide for that http://www.jotform.com/help/259-How-to-perform-calculation-in-the-form.

Feel free to make changes on the form like on the images and the formula. You can change the images within the radio button options on the top toolbar.
