I'm receiving no data in e-mail notifiers. What I get is subject: "(TEST) Re: {form_title}"

and only "undefined" in the body. I've deleted and re-created default notifiers with no luck. Please help!

My end goal is to re-format the tables in the e-mail in order to print the filled form exactly as shown. It appears this should be possible, but I cannot seem to make it work.

Can you please confirm that this was your form http://www.jotform.us/form/60266424891156 that was having the issue?

I checked the form and I noted that it was currently an ecrypted form .

Perhaps the encryption feature was the one causing the problem while you were modifying the format of the Email template.

Can you try first disabling the encryption and modify your form as needed. Save the form, then enable encryption.

Let us know if this process did not make any difference.

I have inspected email Notification settings of form you provided and body of email Notification is empty, it only has "undefined" word. It seems that you have deleted table inside of email Notification. Because of that you are receiving "undefined" emails.

To resolve this you can simply create new Email Notification and delete old one. When you create new Notification all fields and values of fields will be automatically added inside of your Email Notification.

If you want to print your form exactly as it is when viewed, you can to that from submission page on your account or by adding edit link inside of your Notification, as it is described inside of this guide: http://www.jotform.com/help/335-How-to-print-a-form-exactly-as-it-is-when-viewed-but-with-the-user-entered-data

It is not possible to style email Notification to look exactly the same as online form.

When I inspected your form I also noticed that Autoreponder emailis not set correctly, You did not set Sender email and Recipient email inside of your Autoreponder email.

I would suggest you to set Sender email to noreply@jotform.com and Recipient E-mail should be the "Email" field of your form.

Inside of this guide you can find how to create Autoreponder email: http://www.jotform.com/help/26-Creating-a-Form-Autoresponder

This guide can also help you to properly set your email Notification and Autoresponder:
