Why is my form disabled?

Whenever I try to preview or publish my form, it says, Form not found; This form is disabled. I don't know why it says this, as I have it enabled.

Upon checking your account I found that it's suspended by our anti-phishing system. We have automated anti-phishing system which scans all forms and mark them with probability of being used for phishing activities.
