Why has my form disappeared from my shopify site?

    Hi, I used to have a jotform working on my Shopify site but it has disappeared!  Please help!

    I checked on your issue with your form disappearing from your shopify site but it may have been a temporary problem.

    Currently I am unable to replicate this problem since I can see your form on my end.

    Can you please check it again on your end and let us know if this is still happening for you?

    If it is then can you please also tell us what web browser and browser version you are using?
