You have provided me with form submission's screenshot, if you want to create form you will need to do the following.
First you need to create a form.
You can do this by going to MY FORMS. There you need to click on Create form button. When you click on Create form button you need to choose Blank form:
When you click on Create Blank form button you will be redirected to Form builder.
In the Form builder you can find all the tools you need to create the form you need.
Text field is the tool for smaller text sizes, i recommend using it for smaller texts (First name, Last name, Organization name, etc.)
Text area is used for larger text. You can use this tool for Up to 150 word bio or for any text you have that is larger than 20 characters(Bio, Descriptions, etc.).
You can limit the minimum text needed in the text area:
If you have input that has multiple predefined choices you can use dropdown list. You populate the fields in drop down list by clicking on options button. You need to input options separated by new line.
The alternative to dropdown list is radio button.
Drop down list and radio buttons are used when you need to force user to choose one option.
If you want to enable user to choose multiple options you can use check boxes:
It is important to enter labels correctly since Jotform uses them in submissions
