I am currently creating a RGA form to embed on our website.  I want to be able to use a fillable table similar to the one in the image I have attached.  Is this possible?  If it's not I would like to know how I can make multiple text boxes run across the same line, like the Name field that has 'First Name' and 'Last Name' text boxes.

Depending on how you want your data to be processed there are few options that you could use:

    1. Configurable List widget

        - How to Set Up the Configurable List Widget

    2. Spreadsheet widget

    3. Shrink option

        - Form Field Positioning

        - How to Position Fields in JotForm

Based on the headers in your table, I would presume that the Configurable list is the best bet. The Spreadsheet option would be the second bet if you wish to have some kind of calculations done as well.

The shrink option would allow you to create the table as well, but it would be quite complex to set up and any change would mean that you would need to re-adjust your CSS code, making it hard to maintain and to use - still it is possible.

To use it you just need to right click on the field and you will see a new menu open that will allow you to shrink the field, or accessing the same menu by clicking on the field and then clicking on the gear wheel icon.
