How do I Integrate Jot Form Questionnaire with clickfunnels?

Hi all,

I have created a form and now need to incorporate this into clickfunnels. 

a) How do I do this or is there a code I can get to embed?

b) Where does the response of the form go when it is submitted. I use mailchimp. Is there a way to send the information direct to my email?

a) To embed your form to a Clickfunnels.com page, you would need to use the iFrame code. Here's a guide on how you can get the iFrame embed code: Getting the Form iFrame Code.

This code would need to be added in a custom HTML block:

b) When the form is submitted, the form responses will go to your jotform account. To view the responses, please follow this guide: How to view Submissions.

You can also receive a notification of the submission to your email. Simply follow this guide to setup email notifications: Setting Up Email Notifications
