This is the second time where my form has been disabled and not active.
Can you please solve this problem.

I can only presume Sharon that you mean the form that you can get to by looking at the Clients menu in the main navigation? In such case this would be the form: https://form.jotform.com/60185509629260
I do suggest to always let us know how to access the form, or to let us know the link to the same so that we can be sure that we are looking at the right place and properly assist you with the same.
I have now reactivated your account. Please do note that since you are using Facebook ID on your forms and you leave the title of the forms as "title me" this is something that the system will pick as bad, which is also the reason why from time to time you might get your account suspended - depending on the other activity of account and forms.
