I am trying to use the PDF designer to customize the pdf reports that can be sent along with an email notification but it's all messed up. None of the texts/frames are showing up correctly, both in the preview and in the actual pdf sent out.

I cloned your form and tested the PDF in the PDF Designer. I can't seem to replicate your issue as I was able to customize the PDF Design without any issue.
Can you try resetting the PDF design to its default view to see if that fixes the issue? To do so, simply go to Submissions > Get PDF > Customize PDF and click Reset.

elements with additional text and customize it any further than
rearranging, it gets all messed up when I reopen it or when I look at the
preview mode.

Unfortunately I was unable to recreate the issue in my test form. I added some extra text and rearranged the fields using the PDF designer and the design was still displayed properly.
You may try refreshing your form cache to see if that fixes the issue. Here's how: How to Clear your Form Cache. Once the form cache has been cleared, you can try editing the pdf design again.
