It is because you were asking for username and password on your forms, it is against of our terms of use, this is the reason why your forms have been disabled, I have removed those fields in order to restore your account, but please follow this advice, if you ask for information that is not allowed for JotForm your account will be deleted, read our terms on this link: http://www.jotform.com/terms/.
