Form Disabled. Please help ASAP

Please help with this. I was not sure what did I do wrong,  my form is keep saying when I try to submit.

I checked your account and it was in 'Autosuspended' state that happens automatically, if our system suspects that some forms are potentially phishing.

I re-activated your account so it should be working now.
