I can't see my forms when I log in.....

I have checked you account lisapiano101 and you have only one form http://www.jotform.com/form/10181711613  on that account.

I have also checked website you provided and forms that I found on your website, are created on account lisapiano3625. It seems that you have logged in to wrong account. 

You can login to your account with email address or with username. If you have forgot your password you can reset your password:

1. Go to jotform login page

2. Click on the login button on top

3. Click the "Forgot Password?" button

4. Provide your username or email, and click on send reset button.
