    Why is suspended my account?

    I created a Pre-Trip Checklist form and embedded it onto a website for someone in the field to test it out. After testing, he requested that I then create a Save option so he can fill some now and come back to it later. I didn't know how to do that so I searched for the answer. I came across a tutorial on How to create a Save and Continue form. I was a little confused at the directions and decided to check for a demo. I then completed the demo so I could see how the form actually worked and received an email from an Elton Cris. Shortly after that my account was disabled after that. Is there anyway to recover my account and all of the forms I created, especially this one?

    Seems like it is because of this form: http://www.jotform.us/form/60717815161151

    The title is "Log in", and our anti-phishing system could cataloged as a phishing form, because of the name, I have checked it and seems clear, you are not asking for information that is not allowed in JotForm, I have re-activated that account, but please, rename that form in order to avoid get suspended your account again.

    Wow, thank you so much for such a speedy response and for re-activating my account.

    What should I label this form as because I'm trying to create a Save and Continue form. Did I do this correctly? Am I on the right path?

    You're welcome, yes like today is not a busy day we are able to assist you very quickly, I checked your form now and seems like the name it is correct, it will not be cataloged as phishing form and now you will avoid to suspend your account again.

    I finally completed the Save and Continue form, so I thought. When I tested the form to see if it was working, I received the continue email. Yayyyy. However, when I went back into the form to complete the rest or pick up where I left off, none of my previous responses were in the form. Can you please tell me what I did wrong?

    Its been brought to my attention AGAIN that my account was suspended AGAIN for phishing. This is really confusing. Can you please provide me with some "Safe Titles" to use when creating Save and Continue forms so this doesn't happen again. I'm following your steps when creating the first page but your steps aren't clear on how to correctly title them, especially if you need more than 1.

    Is there a way to have just one first page for all of my Save and Continue forms.

    This suspension is really hurting us right now and its all due to ignorance on my part and unclear directions on Jotforms part. My guys are unable to complete there paperwork as a result. 

    I would love your assistance in this matter.

    I checked your account and I see that it was again autosuspended by our system. I review your forms and I see that the form titled "Personal Info" is asking for credit card numbers using plain form fields. 


    Please note that we do not allow users to create forms asking for sensitive information like credit card details and bank information. Please check our terms of use page. If you would like to ask payment from your user, it is a must to use our payment integrations.

    We can re-activate your account, but we will need to delete the form fields that violated our terms.

    I totally understand and I will review the Terms and Conditions. I am ok with you removing the card info section, as well. I apologize for my ignorance. How long will it be before I can log back in and the suspension is lifted?

    I have reactivated your account and deleted the said form fields. Please do check your other forms and see if there are form fields that may violate our terms. JotForm is quite strict on this as we do not want the forms collecting sensitive information as this mimics a phishing page, the information can also be used for identity theft and other illegal activities.
