i have created a form but i don't know why it doesn't work. can you help me?

I checked your account/username Tici and I found that it was temporarily disabled by the Phishing Filter due to mistaken form on the account.
I have now corrected this and reinstated your account. You may login back again to jotform and can see your forms now active again in your My Forms page.
Let us know if issue still persist.
