Pre-populating the form with data

I want to send a link to a customer that will open a customer survey.  The link needs to be able to pass along information to be used on the form.  In this case, the jot form would have to automatically know the store number and SCR number (phone call identification number) somehow.  

It is possible to pre-fill the form with data by using the URL parameters. Please check this guide: Prepopulating fields to your JotForm via URL parameters
