How to disable the Autofill feature?

This happens even after I've closed down the browser - my other forms don't do this.  The form in question can be found on this link http://www.cleantech-norfolk.co.uk/wheeliebincleaning.asp

I'm using Firefox and Windows 10.

This is because you have enabled the Auto Fill feature, to disable it you only need to click on Preferences-> Advanced Settings-> Edit Auto Fill Settings:

Once you are there you only need to disable the option.
