Why my thank you page is showing file not found?

     the Adult Team Information Form after clicking SUBMIT shows FILE NOT FOUND (404 Error), but the information is still being submitted.  

     

    Please help.

    This is because you are using a custom thank you page, it means that you are redirecting your submitter to a different URL rather than the one by default in JotForm, then seems like the file was deleted and this is why you are seeing this message, I would suggest you to check the URL and re-upload the file if possible.
