    This past week our website was tagged with the phrase, "This site may harm your computer". In conversation with my web hosting provider, the issue appears to be a result of the following;

    <script src="//cdn.jotfor.ms/jotform.jgz?3.1.110" type="text/javascript"></script>

    <script type="text/javascript">

       JotForm.init(function(){

          $('input_3').hint('ex: myname@example.com');

       });

    </script>

    <link target="_blank" href="http://cdn.jotfor.ms/jotform.cssgz?3.1.110" rel="nofollow noopener" rel="stylesheet" type="text/css" />

    <link type="text/css" rel="stylesheet" target="_blank" href="http://www.jotformpro.com/css/styles/pastel.css" rel="nofollow noopener" />

    <style type="text/css">

        .form-label{

            width:150px !important;

        }

        .form-label-left{

            width:150px !important;

        }

        .form-line{

            padding:10px;

        }

        .form-label-right{

            width:150px !important;

        }

        .form-all{

            width:500px;

            background:#FFFFFF;

            color:rgb(82, 75, 58) !important;

            font-family:Tahoma;

            font-size:13px;

        }

    </style>

    I was not able to load the URL that you provided. Which embed method are you using?

    Please, try using the iFrame method: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code

    Once we have the link to the website where the form is embedded we will able to make some tests and find what is causing the issue.
