Do you mean that you wish to close down your jotform account completely and also delete it as well per your request?

Pardon, I see you had a subscription previously. So I just wanted to check with you first to see if that is maybe what you meant to close instead? Please confirm or correct me.
I can confirm that your subscription was cancelled and you are currently scheduled to downgrade back to a free account on 3-25-2016.
If you meant otherwise then let us know.
