Why can't my form have a place for affiliate to choose a "Username" and "Password"? I need this so that I can't make their account.

I need my form to have a place for affiliates to choose a "Username" and "Password" by entering it into my form, but it seems you do not allow this. Can you help me to accomplish this?

Unfortunately, it is not possible to have fields like username and password on JotForms. Such forms are considered as phishing forms and associated accounts are immediately suspended. Please refer to our Terms of usage for more information on Phishing forms.
