I would like to have the text box printed

I need to have the form printed out in the email with the text box of the volunteer's affirmation.  how can I do this?

I have checked your form and the notifier set in it. To my understanding, you would like to have the answer to the field "please print your name as this will denote your signature, thanks in advance!" included in the email notification you receive when someone submits your form, is that correct?

I have created a clone version of your form and noticed that it should be included base on your email notification set up.

If this is not what you are referring to, kindly give us more information or explain in further details what you are trying to achieve.

Hi Mike, I appreciate your response. but.... the large text box with the volunteer's 'affirmation' or the text box before the signature required.... the large one....how can I get this to print in the email so that it would be a signed document for the pet rescue? thanks in advance....I am totally new to this, and the base email notification set up is not totally understandable. do you have the availability of a phone consult? I realize you are a free website, as Homestead referred me to you just this afternoon.....

I, (named above) am requesting a volunteer position with the A HUMANE SOCIETY OF CENTRAL FLORIDA PET RESCUE, INC. also, known as AHSCFPRI. I agree to read and follow the rules and guidelines of the organization. I agree not to hold the organization, any director, employee, board member, or any individual personally or otherwise responsible in the event that I sustain personal, financial, emotional, or property loss/damage while serving the organization. I agree to follow the supervision of all persons involved in volunteer management. I understand that, as a volunteer, I am an important representative of the AHSCFPRI and must do my best to represent this organization in a manner that is consistent with its articles, bylaws, guidelines, and philosophies. Hello Deborah,

If I understand your requirement correctly, you want to display the "Consent" texts to be displayed in the submission email. Is that correct?

Please be noted that headers and text fields are not added in the submission emails. You will have to manually add it in the email template of your form. Please check the screenshot below:
