    How to add a percentage of a total amount to a total?

    I want to put 5 fields of multiple choice and each of these fields should add 2% of the total value of the product that will be filled by the customer, how do it?

    I would like to confirm that I have understood you correctly.

    You wish to have 5 multiple choice questions on your form. As each is filled out it will add %2 of the current total to the total amount?
