Form is messed up on Safari

    My form looks good on pretty much all desktop browsers except Safari for Mac. I am running OS X Yosemite version 10.10.5 and the most current version of Safari for Mac. Please see screen shot.

    Any help would be appreciated.

    Can you please try first re-embedding the form https://www.jotform.us/form/52671061023141 on your website  using its Iframe embed code.

    Follow this user guide: Getting-the-Form-iFrame-Code

    Make sure to remove/clean first any previous script embedded code of the form on the website before you re-embed using the Iframe embed code.

    Let us know if issue still persist even after doing this process.
