Hello hugs4kids,
I did check your webpage where you have embedded your form and found that you have used yoru form's full source code. Have you made any changes to your form recently?
I would suggest you to please grab your forms iFrame embed code and use it to embed your form in your webpage. That should solve your problem. The following guide should help you how to grab your form's iFrame embed code: http://www.jotform.com/help/148-Getting-the-Form-iFrame-Code
Hope this helps.
Do get back to us if you have any questions.
Thank you!

Hi Ashwin,
I have tried that but unfortunately, there are too many issues with the style that cannot be controlled. If you look at the page again, I have got both the iframe and my version up at the moment and you can see the difference - one fits my corporate style guide and one doesn't.
I have only made style changes to the html so not sure why my version doesn't work.
Cheers,
Adam

I cloned your form to check for any issues with the required fields. I noticed that the Mobile Responsive Widget was set to Required. This might be the reason why you get "Incomplete Values" when you submit the form. To fix the issue, I suggest you turn off the Required option of the Mobile Responsive Widget.
Do let us know if you need further assistance.
Regards.
