I would like a check box selection or whatever works where the day of the week can be selected and then the user is prompted to enter the times.
For eg. please see uploaded document

I think this could be achieved by using the Configurable List widget: http://widgets.jotform.com/widget/configurable_list.
You will be able to add a check box and a date input on the same line, here is a guide to configure the widget on your form: http://www.jotform.com/help/282-How-to-Set-Up-the-Configurable-List-Widget.

Okay Thank you. I started however I am having difficulty with how to get the text area part to display on each row instead of going across in the column.

I need it to display like below: (or it can go across whichever way works). The days of the week would be check boxes and the area beside them need to be textboxes.

I believe what you wanted is the same as what my colleague, Ben, have explained on this thread, Can-we-combine-checkbox-with-a-textbox. However, that was done without the help of the Configurable List widget.
