I didn't replicate the same issue with you on the form "Online proxy voting form". However, if you are using your form by embedding it on a webpage, I suggest you to re-embed it with iFrame embed code after editing it.

Also, I removed your form caches on our system, so you can try to remove yours on the browser you use before retrying and please, don't forget to let us know the results.
