is it possible for me to code this form http://wefinancecars.ca/div/application/?start=2 to send into jotform platform?
would i have to create a form in jotform with the same field names first ? and alter the souce code

This should be possible, but it might be tricky and you'll probably need to hire an expert CSS developer or a custom script that would replicate the effects or styling of that form.
