Form is not working

A pop keeps appearing that says "form is not available." Please help ASAP

I'm checking your form titled "Wag Orientation Quiz", and it looks accessible. 

 

However, when I submit the form, I got the error. Here's a screencast:

 

I presume the error is showing because you have set the submission limit count to "2". You won't be able to receive submissions due to this limitation.

 

Could you please try remove or set the "Submission Limit" to blank, save the changes and make a test submission. See if that works.
